I agree to the following:

1. To keep the animal(s) in my possession, to care for in such a manner as to meet with the approval of tlc Rescue.
2. Not to declaw the animal(s) without the consent of tlc Rescue. If any animal is declawed without first discussing and gaining approval from tlc Rescue, a donation of $2000 will be due immediately. tlc Rescue is allowed to contact any vet to whom adopted animal(s) is taken for vet services with regard to any declaw.
3. Not to sell, trade or dispose of said animal(s) in any way but to return to tlc Rescue, if at any time I desire to relinquish custody, making no charges of any kind for licensing, care, food, or other service.
4. That said animal(s) shall not be used for medical or any other experimental purpose whatsoever.
5. To have the animal(s) spayed/neutered no later than 6 months of age and will provide proof of such spay/neuter upon request.
6. To respond to any reasonable request from tlc Rescue to examine and make inquiry about said animal(s) at any time, and if conditions are not found satisfactory, to return said animals(s) to tlc Rescue.
7. To release tlc Rescue from any liability which the animal(s) may incur while in my care or possession.
